CARF accreditation represents the highest level of recognition awarded to an organization and shows substantial conformance to CARF standards. An organization receiving a three-year accreditation has put itself through a rigorous peer review process. It has demonstrated to a team of surveyors during an on-site visit its commitment to offering programs and services that are measurable, accountable and of the highest quality.

These areas have held the designation since 2014 and 2018, respectively. By achieving CHAP Accreditation, Optimae has met Medicare Conditions of Participation and is certified as a Medicare provider. As the first accrediting body in home health care, CHAP is the recognized leader in Home Health Accreditation, with more Medicare-certified home health providers than any other accrediting body recognized by Centers for Medicaid and Medicare Services (CMS). CHAP’s guidance, education and resources support its providers as the organization strives to deliver the best patient care.
CHAP accreditations follow an in-depth self-study and multi-day site visit with a CHAP surveyor that includes field visits with nursing and home care aides, review of all HR files and review of charts, policies and procedures.
CHAP Accreditation demonstrates that Optimae meets the industry’s highest nationally recognized standards. The rigorous evaluation by CHAP focuses on structure and function, quality of services and products, human and financial resources and long-term viability. Simply stated, adherence to CHAP’s standards leads to better quality care.
This accreditation process requires 100% compliance with all accreditation standards and does not offer partial accreditation.
To become accredited by QUAD A, an outpatient therapy facility must demonstrate a strong commitment to consistent, fiscally responsible patient services by complying with the most rigorous set of applicable health care guidelines available in the nation. A facility is officially recognized only after passing an annual survey conducted by qualified QUAD A surveyors and meeting QUAD A’s strict requirements in categories including general environment, personnel, general safety in the facility, medications, medical record, quality assessment and quality improvement.
According to QUAD A, “QUAD A facility surveyors are board certified medical specialists trained to perform a thorough and complete facility inspection. To earn its accreditation, Optimae LifeServices passed inspection on hundreds of important criteria concerning patient safety.”
Per the Iowa Department of Health and Human Services’ website:
“Iowa Medicaid has had a robust HCBS quality oversight system in place for over thirty years. This quality oversight system includes regular, ongoing review of providers of HCBS waiver and Habilitation services. Iowa Medicaid’s QIO HCBS team is responsible for all quality oversight activities for the HCBS waiver and Habilitation providers, including the review and assessment of HCBS settings.
“Quality oversight includes, but is not limited to, annual provider self-assessment, Periodic or Certification Reviews, CNRS Periodic Reviews, Focused Reviews and review of incidents and complaints (also known as Targeted Reviews).”
